Job description
To provide a Social Work Service to a defined patient caseload and their families and carers. To work in collaboration with the Multidisciplinary team...
To plan and arrange subsequent mental health care based on identified needs.
Provide profession-based support and advice across the MDT and within multidisciplinary meetings.
To lead and possess a full understanding of the discharge process in low secure settings and the knowledge and skills to apply effectively in practice to ensure a safe and timely discharge takes place.
To ensure that the estimated discharge date is identified at admission, during reviews where possible. Putting in place early interventions to remove barriers to prevent patients discharge process being delayed
To negotiate clinical input from other services within SHSC to ensure the
provision of a clinically appropriate and multi-disciplinary approach
To develop close working links with other mental health teams and agencies
to enable swift and appropriate transfer between services
To share assessments and updates on the person's health status, including medication information with both hospital and the community based teams
To contribute to the development of time limited individual care plans in
consultation with patients, families/carers, community teams, primary care
and other appropriate service providers
Engage in functional Assessments, Goal setting and group work as appropriate
To be the main contact person and be confident at building, developing and fostering relationships with carers families and friends.
Professional
Maintain Social Work England registration and adhere to professional guidelines and code of conduct.
Work within the framework of relevant policy and legislation both local and national.
Ensure that clinical practice is in line with relevant regulatory body Code of Conduct.
Ensure that practice is evidence based and participate in research/audit when required.
Participate in mandatory training and training / education relevant to the post.
Keep abreast of national developments in mental health care.
Liaison & Networking
To develop and maintain close working relationships with existing mental.
health teams, primary care services, Sheffield City Council, and other key stakeholders.
To promote the relevant team through participation in local and national
arena.
Education and Training
Act as mentor to learners, keep up to date with relevant course and curriculum developments, and maintain sign off status.
Participate in training relevant to role.
Promote insight and understanding of mental health issues to service users, carers, and other agencies.

Qualifications
Essential
- Registered Social Worker with relevant professional registration with Social Work England.
Knowledge
Essential
- Extensive Knowledge of MHA including Part 2 and Part 3
- Demonstrate a detailed knowledge of current Social Work practice
- Commitment to enhancing own knowledge and skills through continued professional development
