An exciting opportunity has arisen for an experienced and motivated physiotherapist to join the Cardiff and Vale Supportive Care Service. The Supporti...
- Provide specialist physiotherapy assessment and intervention for patients with advanced liver, renal, respiratory and cardiac conditions.
- Assess and manage symptoms including breathlessness, fatigue, reduced exercise tolerance, deconditioning, mobility problems and functional decline.
- Deliver personalised rehabilitation programmes to support independence, function and participation in meaningful activities.
- Work collaboratively with patients to establish realistic goals that reflect their priorities and preferences.
- Contribute to holistic multidisciplinary assessments and care planning.
- Support patients to self-manage symptoms and optimise physical wellbeing.
- Work closely with consultants, specialist nurses, occupational therapists, psychologists and other members of the multidisciplinary team.
- Participate in service development, audit, evaluation and quality improvement initiatives.
- Support the education and development of colleagues, students and support staff.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
