- You will be leading the delivery of the Healthy Child Programme completing the 5 mandated contact with the addition of the new 4 year health and development review to support school readiness and transition.
- You lead and will be responsible for a skill mix team delivering public health service initiatives to a defined population.
- You will work as part of a team that includes Community Public Health Associates, and Community Public Health Nurses with additional support from experienced Locality Team Leaders delivering a service alongside SCPHN SHN and the FNP in 0-19 locality teams.
- You will deliver services to children and families within homes, and community settings.
- Clinical activities will include assessing, planning, implementing and evaluating family focused care and delegating to CPHA and CPHN when appropriate. You will develop multi agency relationships and work closely with other agencies including the voluntary sector.
- The role will require the post holder to adhere to Safeguarding policies and actively engage in safeguarding processes.

Qualifications
Essential: - First Level Registered Nurse or midwife; - Specialist Community Public Health Nurse (health visiting)
Desirable: - Qualified mentor; - Teaching skills and/or qualification; - Nurse Prescriber; - Post-graduate qualifications
Experience
Essential
- Experience of inter-agency working
- Evidence of current knowledge of National and Local policies and relevant research and the ability to relate it to practice
- Reflective practitioner
Desirable
- Clinical Supervisor
- Presentation skills in line with organisational competencies
- Evidence of a recent managerial experience
- Experience of facilitating clinical supervision, learning and development
Skills
Essential: - Evidence of leadership skills; - Demonstrates knowledge and skills of IT
