The post holder will support the assessment, planning and delivery of person-centred care under the guidance of a supervising clinician, ensuring care is provided in line with clinical governance standards. They will undertake and accurately record physical health checks, including supporting individuals during clozapine titration and promoting their overall physical wellbeing. The role also includes leading on carer support within the team, ensuring carers are identified, involved and provided with appropriate information and support while individuals are receiving care from the service.

Qualifications
Essential
- Foundation Degree in Mental Health or related subject.
- OR Level 3 Qualification in health-related area and two years' experience in a care role.
- Significant experience in role delivering care in a mental health setting.

- Subject to reasonable adjustments in respect of any disability, candidates must possess a valid driving licence and be able to drive and have the unrestricted use of a car or motorcycle for work at all times including community based work.
