Social Worker - Learning Disabilities TeamLocation-West Midlands Duration - 12 weeksHours per week- 37 hoursHybrid Working Pay Rate-28.40We are seeking an experienced and passionate Social Worker to join a dedicated Learning Disabilities Team in the West Midlands. This is an excellent opportunity to make a meaningful impact, supporting adults with learning disabilities to achieve their goals, maximise their independence, and live safe, fulfilling lives within their communities.Working within a supportive multidisciplinary environment, you will play a key role in delivering high-quality, person-centred social work practice, ensuring individuals are empowered to make choices and have control over the support they receive.The RoleAs a Social Worker within the Learning Disabilities Team, you will manage a varied caseload and work closely with individuals, families, carers, health professionals, and partner agencies to achieve positive outcomes.Key responsibilities include:

Undertaking strengths-based assessments in line with the Care Act 2014.Developing and reviewing person-centred care and support plans.Supporting adults with learning disabilities to maximise independence and wellbeing.Completing Mental Capacity Act assessments and Best Interest decisions where required.Managing safeguarding enquiries and promoting positive risk-taking.Working collaboratively with health colleagues, housing providers, advocacy services, and community organisations.Supporting transitions, hospital discharges, and long-term support planning where appropriate.Ensuring individuals have access to the right services, resources, and opportunities within their local communities.Maintaining accurate, timely, and comprehensive case records.Contributing to a culture of continuous improvement and high-quality practice. Essential Requirements

Qualified Social Worker (Degree, DipSW, CQSW or equivalent).Registration with Social Work England.Experience working within Adult Social Care.Sound knowledge of:

Care Act 2014Mental Capacity Act 2005Safeguarding Adults ProceduresHuman Rights ActEquality and Disability legislation Strong assessment, risk management, and care planning skills.Full UK driving licence and access to a vehicle (where required) This role is exempt from the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975.
