About the roleAbout the roleWe are looking for a social worker with experience of working in Continuing Health Care services to join the Complex CHC Team. The role is for a Full time Social Worker,37 hours a weekAs a Social Worker you will be supported by our team’s Principal Care Manager to understand the role and be provided with continuous professional development.The role is a fast-paced role, and you will work directly with individuals, nurse assessors and other health professionals , care providers, and other stakeholders.You will be attending in person meetings in the community , working remotely and in the office for duty work. N.B. All IT equipment will be provided.What will you be doing?What will you be doing?You will be working in line with key legislation such as the Care Act 2014 and frameworks such as the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care 2022. You will be attending multi-disciplinary meetings to represent Adult Social Care to discuss health needs and make decisions on behalf of the local authority. You will be gathering information about the person using the 3 conversations model of approach , which is person centred and strengths based.You will ensure the person’s voice is heard throughout all your work with them .You will be observant, actively listen and apply professional curiosity to your practice.You will be able to identify the need to apply safeguarding and to raise concerns appropriately.
