- Carry out planned care and training for a group of patients without direct supervision.
- To coordinate the PD and HHD training program based at Home Therapies training unit and/or satellite units and external companies.
- Plan, organize and deploy patients into the PD and HHD training program according to suitability.
- Prioritize and manage patient workload as per referral of patients in liaison with the Head nurse Use initiative where training targets are not being met, discuss with Head Nurse, Home therapies team at earliest opportunity and agree revised action plans.
- Assess the needs of patients, relatives and carers and provide them with information as requested. Liaise with the dietician, counsellor, technical services, transplant team and refer to patient seminars as appropriate.
- To keep up to date with current developments in nursing and ensure evidence based practice.
- Develop new skills and knowledge pertaining to the specialty as advised by the senior home therapies team.

Education/ Qualifications
Essential
- Registered Nurse
- Evidence of continuing professional development demonstrating advanced assessment skills
- Relevant specialist course
- Relevant post registration renal course (ENB 136/ or equivalent )
Desirable: - Graduate BSC/MSC; - Teaching Qualification (ENB 998/Slice or equivalent experience)
Experience
Essential
- Renal experience (> 6months)
- Extensive experience both in PD and in HHD - acute and chronic.
- Experienced, competent, knowledgeable and skilled in handling dialysis machines, e.g., Claria APD, NxStage HHD machine, etc.
Desirable: - Previous work experience in PD or Home HD; - Experience of being a Team Leader / Deputy In charg
Skills/Knowledge/ Abilities
Essential
- Organization and management skills.
- Excellent teaching skills.
- Ability to conduct and lead the PD / HHD training program
- Ability to work effectively under pressure to meet the needs of both training programs.
- Ability to solve problems.
- Ability to perform clinical activities to an advanced level with advanced assessment skills.
- Competency in medication administration including IV drugs. IT skills including use of email and office packages.
Desirable: - Knowledge of budget management; - Knowledge of rostering; - Knowledge of Health & Safety management; - Phlebotomist; - Cannulation
