- Deliver specialist mental health assessment, rehabilitation and intervention within community settings.
- Support patients to achieve their recovery and rehabilitation goals, promoting independence and wellbeing.
- Undertake physical care assessment and treatment within scope of own professional practice
- Provide specialist mental health advice, guidance and support to Community Therapy Service colleagues.
- Work collaboratively with Community Mental Health Teams, primary care, social care services, and partner agencies.
- Support effective risk management, clinical decision-making, and care planning.
- Contribute to service improvement and the ongoing development of integrated community care pathways.

Qualifications
Essential: - Occupational therapy/Physiotherapy degree or equivalent; - HCPC registration
Desirable: - oExperience in community mental health, rehabilitation or integrated care services.
Experience
Essential: - oExperience supporting people with co-existing physical and mental health needs.; - oExperience of mental health and risk assessment.
Desirable: - Good knowledge of rehab & community services
Skills
Essential
- Excellent written and verbal communication skills
- Good knowledge and skills in elderly care/rehabilitation and have specific clinical expertise in elderly care and rehabilitation
Desirable: - oExperience delivering training, education or reflective practice sessions.
