Manage and coordinate a caseload of adults with severe and enduring mental health conditions.
Act as Lead Professional, developing and reviewing person-centred care plans and risk management plans.
Undertake comprehensive mental health, risk and needs assessments.
Deliver recovery-focused therapeutic interventions that promote independence, choice and wellbeing.
Work collaboratively with service users, carers, community services and multidisciplinary teams to ensure effective care coordination.
Maintain accurate clinical records and contribute to service performance and outcome monitoring.
Prepare professional reports and attend multidisciplinary meetings, reviews and case conferences.
Provide mentorship, support and supervision to students, trainees and junior staff.
Participate in safeguarding, clinical audit, quality improvement and service development activities.
Ensure care is delivered in line with relevant legislation, professional standards, Trust policies and best practice.

Qualifications
Essential
- Registered Mental Health Nurse (RMN) with current NMC registration.
- Evidence of ongoing professional development and commitment to maintaining professional competencies.
Desirable
- Ability to demonstrate knowledge of current mental health legislation, policies and best practice.
- Experience of working with adults with severe and enduring mental health needs within community or inpatient mental health services.
- Strong assessment, risk management and care coordination skills.
- Excellent communication, organisational and multidisciplinary working abilities.
- Competent in maintaining accurate clinical records and producing professional reports.
- Mentorship, practice assessor or supervision qualification.
- Additional training in psychosocial interventions, risk assessment, safeguarding or therapeutic approaches.
- Experience of care coordination and working within a Community Mental Health Team (CMHT).
- Knowledge of safeguarding, Mental Health Act, Mental Capacity Act and Care Programme Approach processes.
