The successful post holder will deliver patient centred, integrated respiratory care as part of a seven day service, supporting admission avoidance and contributing to the development of the Integrated Airways Virtual Hospital in partnership with the Hospitals Trust.
You will develop and apply specialist skills in the management of patients with respiratory conditions including COPD, Asthma, Bronchiectasis, OSA, and ILD, as well as supporting patients who require home oxygen therapy.
This patient centred role includes
- o Undertaking comprehensive respiratory assessments for patients with complex needs and co morbidities, including physical examination, clinical analysis, and the development of individualised, evidence based management and treatment plans.
- Implementing specialist care programmes such as home oxygen therapy, pulmonary rehabilitation, medication optimisation, and psychological support interventions, in line with national and local guidelines.
- Providing compassionate emotional support to patients and their families or carers living with long term and life limiting respiratory conditions.
- Delivering care through a combination of community home visits and nurse or physiotherapist led clinics across West Hertfordshire.
As part of a large, experienced community respiratory service, you will be supported by a skilled multidisciplinary team including Band 7 and Band 8a clinicians, a Consultant Respiratory Physiotherapist, and Respiratory Consultants.

Education/Qulifications
Essential: - Nursing/ Paramedic/ Physio Degree qualification or equivalent; - Current NMC /HCPC registration; - Teaching/Mentorship qualification or equivalent experience

- Experience of working with respiratory patients
- Experience of working within acute or community setting.
- With knowledge of relevant of National guidelines in Respiratory
- Demonstrates ability to develop new initiatives.
- Demonstrates ability to managing change
Skills and Knowledge

- Good effective communication skills, verbally and in writing.
- Ability to make decisions and prioritise actions.
- Evidence of expertise in clinical skills
- Experience of teaching others.
- A working knowledge of clinical governance and evidence-based practice.
- Ability to develop and maintain team working.
- Ability to learn new skills and develop skills in physical assessment, spirometry, developing self-management action plans, capillary blood gas sampling, Feno.
- Ability to work under pressure and to tight deadlines.
- Ability to work autonomously.
- Strong organisational, time-management skills
- To be able to deal with situations which may lead to adjustments within teams' workload.
- Sound working knowledge of guidelines in the field of respiratory care.
- Excellent documentation skills
- Knowledge awareness of recent NHS policies/DoH agenda
- Ability to demonstrate an understanding of the holistic needs of the client group.
- Computer skills i.e., Word, data- bases email, internet
- An eagerness to contribute to quality assurance and audit.
