- Deliver consistently high standards of nursing care with kindness
compassion and respect.
- Assess, plan, deliver and evaluate high-quality, person-centred care.
- Contribute to effective multidisciplinary team working and support a positive

- Participate in rehabilitation and discharge planning to support safe, timely

Qualifications
Essential: - Current NMC Adult Registered Nurse; - Working towards completion of Nurse training programme to obtain registration with the NMC.
Desirable
- Provides learning and development to other professionals within own work area and other clinical areas
- Practice Supervisor and/ or Practice Assessor Trained
Experience
Essential: - Evidence of inter-professional working; - Experience gained in acute, frailty, or community-based nursing roles where complex patient needs are managed
Knowledge
Essential: - Demonstrates knowledge of policies, practices and procedures e.g. NHS national policies, standards, requirements and directions that relate to patient care
