We are hiring several new Senior Reablement Practitioners to join our fantastic service. Joining us as a Senior Reablement Practitioner, you will be doing something that really matters. Working autonomously, you will provide support to people within their own homes, often as a lone-worker. We have roles available in our East 2 area (Elmbridge, Epsom & Banstead, Mole Valley). We are an Occupational Therapy-led service and our community workforce includes Occupational Therapists, Specialist Workers, Team Leaders, Senior

Reablement Practitioners and Reablement Practitioners. We are a multi-disciplinary team who provide short term support for up to 6 weeks. Support is usually provided to adults in their own homes to promote independence and choice by helping them work towards their reablement goals as set out in their support plan. We aim to help improve people's quality of life, maximising their long term independence and enabling people to remain or return to live in their own home within the community. Our team uses a strengths-based approach when supporting people to achieve their goals. We work closely with some of our private providers across Surrey. They are part of our Collaborative Reablement Service (CRS) and are an extension of our Reablement Team. About the Role As a Senior Reablement Practitioner (SRP), you will play a key role in supporting people to maximise their independence and achieve the goals that matter most to them. You will complete initial assessments for people referred to the Reablement Service, working collaboratively with individuals, their families and carers (where appropriate) to develop personalised support plans focused on strengths, recovery and independence. You will support a diverse range of people, including older adults, people with physical disabilities, learning disabilities and/or autism (LD&A), and people experiencing mental health needs. You will work alongside individuals to identify their strengths, aspirations and outcomes, enabling them to build confidence and develop the skills needed to live as independently as possible. You will support people to regain confidence and skills in everyday activities, which may include personal care, medication support, meal preparation, mobility and other daily living tasks. Your role will focus on enabling people to achieve their own goals and reducing their reliance on ongoing support wherever possible. As an SRP, you will regularly review support plans and progress, ensuring that the support provided remains person-centred and responsive to individual needs. You will work as part of a multidisciplinary team alongside Occupational Therapists, Team Leaders, social care professionals and health colleagues, contributing to case discussions and service planning through regular team and MDT meetings. You will also play an important role in supporting the development of the workforce by assisting with the induction of new team members and providing guidance, coaching and support to Reablement Assistants. Following appointment, you will be expected to complete Trusted Assessor Training within six months. This training will enable you to assess for and prescribe a range of community equipment, such as raised toilet seats and bed levers, helping people to live safely and independently at home while working towards their reablement goals. Surrey has both urban and rural areas and locality based social care workers will be expected to have a valid driving licence to drive in the UK and access to a vehicle and be willing to travel across a wide geographical area.
