Practice Nurse

The post holder

will work as an autonomous Practice Nurse, providing high-quality,

evidence-based nursing care to the practice population and supporting the

delivery of safe, effective and patient-centred services.

Clinical Responsibilities

Undertake chronic disease management clinics,

Cardiovascular Disease primary prevention

Carry out annual reviews, monitoring and personalised

care planning for patients with long-term conditions.

Provide advice, education and support to patients to

encourage self-management and healthier lifestyles

Administer and promote national and local immunisation

programmes, including childhood, influenza, travel and routine vaccinations.

Deliver women's health services including cervical

cytology and contraception advice within competence.

Undertake wound care, dressings, treatment of minor

injuries, and removal of sutures and staples.

Ensure accurate, contemporaneous and legally compliant

clinical record keeping.

Work within professional scope of practice and

recognise when referral to a GP or other clinician is required.

Process pathology results as required

Provide chronic disease clinics and Maintain skills and an interest in Diabetes, Asthma, COPD, Spirometry, Coronary Heart Disease, Hypertension, Stroke, TIA and Contraception.

Administrative and professional responsibilities

Participate in the administrative and professional responsibilities of the Practice team.

Attend and contribute to relevant internal and external meetings as

required including practice clinical meetings, nurse team meeting and

care planning meetings

Maintain a personal progress and training plan to keep up to date with

current thinking and developments, as well as identifying own training

requirements, attending appropriate courses

Promote evidence-based practice through use of latest research based

guidelines

Monitor the effectiveness of their own clinical practice through quality

assurance strategies such as the use of peer audit and review

Maintain professional registration, including revalidating as required

Work within the latest NMC Code of Professional Conduct

Record accurate consultation date in patients records in accordance with

NMC guidance and other standards

Administering of injections as per local PGDs

Reviewing and signing PGS and keep up to date with revisions

Keep up to date with pertinent health policy and work with the practice

team to consider the impact and strategies for implementation

Work collaboratively with colleagues both internally and externally

Demonstrate leadership

Use technology and appropriate software packages as an aid to the

planning, implementation and monitoring of care, presenting and

communicating information

Act as a role model in the observance of equality and diversity good

practice

Knowledge and Understanding of Child protection framework/guidelines

Ensure accurate notes of all consultations and treatments are recorded in the patients notes on the clinical computer system as appropriate.

Ensure accurate completion of all necessary documentation associated with patient health care and registration with the Practice.

Ensure collection and maintenance of statistical information required for regular and ad hoc reports.

Assist in the routine recall of patients with chronic diseases.

Assist in the formulation of Practice philosophy, strategy and policy and develop appropriate protocols.

Centre is a friendly, forward-thinking GP practice with approximately 7,800

patients, cared for by an effective, dedicated and comprehensive primary

healthcare team. Our multidisciplinary workforce includes GPs, Practice Nurses,

Healthcare Assistants, Clinical Pharmacists, Physiotherapists, Social

Prescribers, Paramedics, and Reception and Administration Teams.

the MWEB Primary Care Network working collaboratively with Moulton

Surgery, Kingsthorpe Surgery, Denton Village Surgery and Woodview Medical

Centre to enhance patient services and improve health outcomes across our local

population.
