Practice Nurse with Chronic Disease Management Experience

The successful

candidate will be supporting the Practice's chronic disease management

programme, helping patients to manage their conditions effectively, improve

their health and wellbeing, and reduce the risk of complications.

Main Duties of the

The successful

candidate will undertake autonomous clinical practice within their professional

scope and will contribute to the delivery and development of chronic disease

services across the Practice.

The successful

candidate will be a professional, motivated and patient-focused Registered

Nurse who is able to work confidently and autonomously while also contributing

positively to our multidisciplinary team.

You will be

expected to deliver safe, effective and compassionate nursing care to our

registered patient population and to work within your professional scope of

practice.

Depending on your skills, experience and areas of clinical interest, there will be opportunities

to develop your role and take responsibility for specific areas of chronic

disease management and wider Practice Nursing.

Experience

Essential: - Competency in basic nursing duties required for the role.; - Specific nursing skills/qualifications.; - Chronic Disease Management.

Desirable: - Experience of INR, family planning, cervical cytology, immunisation, travel clinic, knowledge of EMIS.

Qualifications

Essential: - Registered General Nurse with current NMC registration.

Skills and Abilities

Desirable: - Understanding of vaccinations and immunisations for children and adults.
