Practice Nurse - North Cardiff Medical Centre

• Provide clinical services as designated by the Nurse Manager/Clinical Governance Lead and Provision of practice Nursing duties
• Work with other health care professionals to monitor, manage and treat long-term conditions, in line with national and local policies and practice needs.
• Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols.
• Work with patients in order to support adherence to prescribed treatments
• Support patients to adopt health promotion strategies that promote them to live healthily, and apply principles of self-care
• Support and manage health needs of women presenting for family planning and cervical cytology consultations.
• Participate in vaccination and immunization programmes for both adults and children.
• Undertake immunisations, injections and vaccinations
• Undertake screening, e.g. blood pressure, cervical cytology.
• Management of medical/nursing supplies, including stock control and expiry checks.
• Maintenance of treatment room and clinical equipment including testing and recording where so defined and adherence to infection control procedures.
• Maintain accurate comprehensive records of all consultations and treatments in the patient record and completion of all necessary documentation associated with patient health care.

Essential

• RN level 1
• Experience of working in Primary care nursing team
• Skills and competencies to include: -
• Cervical screening
• Immunisations child and adult
• Travel vaccinations
• Contraception reviews
• Knowledge and experience of Chronic disease reviews

Desirable

• Post-basic Nursing qualification
• ECDL or equivalent.
• Professional knowledge acquired through training to diploma/degree level
• Appropriate training /experience in chronic disease management and other training as identified by their manager
