Practice Nurse

• Delivering nurse-led clinics for patients with Long-Term Conditions (LTCs), including annual reviews and supporting QOF delivery.
• Providing treatment room services, including wound care, dressings, venepuncture, ECGs, blood pressure monitoring, injections and specimen collection.
• Administering childhood, adult and seasonal vaccinations and immunisations, and undertaking cervical screening where qualified.
• Promoting healthy lifestyles through patient education, NHS Health Checks and preventative care.
• Maintaining accurate clinical records using SystmOne, including appropriate clinical coding.
• Supporting infection prevention and control, maintaining clinical equipment and treatment room stock, and contributing to clinical audit and service improvement.
• Recognising and escalating clinical concerns appropriately and maintaining high standards of patient safety and clinical governance.

• Assess, plan, implement and evaluate patient care within your scope of practice.
• Deliver nurse-led Long-Term Condition (LTC) clinics, including diabetes, asthma, COPD, hypertension and cardiovascular disease, according to your competencies.
• Undertake structured annual reviews and support the achievement of Quality and Outcomes Framework (QOF) and Enhanced Service targets.
• Carry out NHS Health Checks and provide health promotion and lifestyle advice.
• Undertake a range of treatment room duties, including:- Wound care and dressings
• Venepuncture
• ECGs
• Blood pressure monitoring
• Injections and medication administration within competency
• Specimen collection
• Removal of sutures and clips
• Deliver childhood, adult and seasonal immunisations and vaccinations, including influenza and COVID-19 programmes.
• Undertake cervical screening where appropriately trained and qualified.
• Promote self-management and preventative healthcare through patient education.
• Recognise deteriorating patients and refer or escalate appropriately.
• Maintain accurate and comprehensive clinical records using SystmOne, ensuring appropriate clinical coding and documentation.
• Ensure treatment rooms remain clean, safe, well-stocked and compliant with infection prevention and control standards.
• Participate in clinical audit, quality improvement and service development.
• Work collaboratively with GPs, the Nursing Associate, Healthcare Assistants and the wider multidisciplinary team to deliver safe, effective and compassionate patient care.
• Maintain professional competence and undertake relevant continuing professional development (CPD).
• Contribute to clinical governance and maintain high standards of patient safety and confidentiality.

Essential

• Previous experience working as a Practice Nurse within General Practice.
• Demonstrable experience managing patients with Long-Term Conditions (LTCs), including conducting structured reviews and developing/implementing appropriate care plans.
• Experience delivering high-quality, evidence-based nursing care within a General Practice setting.
• Experience undertaking clinical assessment and delivering evidence-based care.
• Experience of treatment room nursing, including wound care, venepuncture and specimen collection.
• Experience maintaining accurate electronic clinical records.
• Experience working independently and as part of a multidisciplinary team.

Desirable

• Experience managing a range of Long-Term Conditions, including diabetes, asthma, COPD, hypertension and cardiovascular disease.
• Experience supporting QOF delivery and Enhanced Services.
• Experience delivering NHS Health Checks.
