• Work as a self-directing practitioner within the managerial structure laid down by the Practice, delivering nursing care to the Practice population and working with GPs and other clinicians to meet patients' medical needs as appropriate.
• Ensure that care delivered to the Practice population is relevant, of a high standard and clinically effective.
• Work as a member of the Nursing Team alongside GPs, Nurse Practitioners, Practice Nurses, HCAs and the wider multidisciplinary team to provide an optimum level of care to patients.
• Work within the framework of the NMC Code and professional standards.
• Support audit activity and quality initiatives, including QOF.
• Maintain and develop their professional knowledge and clinical competencies.
• Be flexible in providing clinical cover across North Elmham and Swanton Morley according to Practice requirements.

• Provide clinically appropriate care and treatment to the Practice population, either independently or in conjunction with a GP or other appropriate clinician, in accordance with Practice policies and protocols.
• Undertake asthma reviews and provide ongoing asthma management. The post holder must be competent and experienced in this area.
• Undertake a range of other Long-Term Condition reviews within their scope of competence, supporting the Practice in achieving relevant QOF indicators.

• Advise patients on a range of health-related matters, referring them to an appropriate healthcare professional when necessary.
• Adhere to infection prevention and control procedures at all times.
• Ensure clinical rooms are kept clean, tidy and appropriately stocked.
• Ensure clinical and emergency equipment is appropriately maintained and any faults are reported promptly.
• Maintain up-to-date Basic Life Support and Anaphylaxis training.
• Undertake ongoing training and professional development and, where required, develop skills in specific areas of long-term condition management.

• Maintain accurate, clear and contemporaneous electronic patient records.
• Be proficient in the use of the Practice clinical system and relevant clinical templates.
• Document assessments, care provided, outcomes and any amendments to a patient's care plan.
• Ensure an appropriate entry is made in the patient's medical record following each consultation or clinical interaction.
• Complete all appropriate documentation necessary for the ongoing care of patients.
• Maintain patient confidentiality at all times.

• Undertake continuing education and training to ensure their clinical practice remains current, evidence based and effective.
• Maintain all required statutory and mandatory training.
• Participate in relevant Practice training and clinical updates.
• Contribute to the education and training of students and other members of the team where appropriate and within their areas of expertise.
• Participate in audit, research and quality improvement activities where appropriate.

• Work collaboratively with colleagues across Elmham Group of Practices.
• Maintain a professional and supportive approach to colleagues and patients.
• Follow all Practice policies and procedures.
• Maintain confidentiality and comply with information governance requirements.
• Work within their professional scope of practice and recognise when referral to another healthcare professional is required.

Essential

• The successful applicant must have:
• First level Registered Nurse qualification and current NMC registration.
• Experience and competency in asthma reviews and asthma management.
• Evidence of continuing professional development.
• Competence in core Practice Nursing skills.
• Ability to undertake long-term condition reviews.
• Ability to maintain accurate patient records, including electronic patient records.
• Ability to work autonomously and use their own initiative.
• Up-to-date nursing and clinical knowledge.
• Excellent interpersonal and communication skills.
• Ability to work effectively as part of a team.
• Good IT skills.
• Understanding of clinical audit.
• Ability to work effectively under pressure.
• A flexible and adaptable approach.
• Ability to travel between practice sites as required.
• Enhanced DBS disclosure.

Desirable

• Previous experience working within Primary Care.
• Long-Term Condition Certificate, Diploma or Degree.
• Independent/Supplementary Nurse Prescribing qualification.
• Knowledge of local and national guidelines and protocols.
• Experience in the management of additional long-term conditions.
