Practice Nurse

• Deliver evidence-based nursing care in line with practice policies and national guidance.
• Undertake chronic disease reviews including diabetes, asthma, COPD and hypertension.
• Administer vaccinations and immunisations.
• Perform cervical cytology screening where appropriately trained.
• Provide wound care, dressings and other treatment room services.
• Support health promotion and preventative care initiatives.
• Maintain accurate clinical records and documentation.
• Participate in clinical audits, quality improvement activities and mandatory training.
• Promote safeguarding, infection prevention and patient safety.

• Provide routine and specialised nursing services including:
• Chronic disease management (e.g., diabetes, asthma, COPD, hypertension)
• Wound care, dressings, and suture / staple removal
• Cervical cytology and women's health procedures
• Childhood and adult immunisations
• Travel health advice and vaccinations
• Contraceptive advice within competence
• NHS health checks and lifestyle interventions
• Undertake clinical assessment where appropriate, assessing patient needs and escalating when required.
• Conduct holistic assessments and contribute to personalised care planning for long-term conditions.
• Deliver evidence based advice to support patient self management and prevention of illness.
• Recognise, respond to, and escalate safeguarding concerns in line with local and national protocols.

• Maintain accurate, comprehensive clinical records in accordance with NMC and legal requirements.
• Adhere to infection prevention and control standards, including safe management of clinical areas and equipment.
• Support the practice in meeting CQC, NHS, and local commissioning requirements.
• Participate in audits, quality improvement projects, and significant event reviews.
• Keep up to date with current clinical guidance and undertake mandatory and discretionary CPD.
• Maintain NMC registration and portfolio for revalidation.

• Work collaboratively with GPs, ANPs / ACPs, HCAs, pharmacists, and administrative teams.
• Provide clear and empathetic communication to patients, families, and carers, including those with complex needs.
• Support junior staff and students, contributing to training and mentorship where appropriate.
• Participate actively in clinical meetings, multidisciplinary discussions, and practice development planning.

• Contribute to efficient day to day running of treatment rooms and clinical areas.
• Assist with stock control, emergency equipment checks, and vaccine storage/monitoring.
• Support implementation of practice policies, protocols, and health and safety procedures.
• Adapt flexibly to changing service needs, including seasonal pressures and public health campaigns.

• Any other duties as required by the nursing team and within the scope of clinical and professional competence of the employee.

Essential

• Registered Nurse with current NMC registration.
• Evidence of continuing professional development (CPD).
• Qualification or recognised training in chronic disease management.
• Competence in immunisation and vaccination delivery, including maintenance of relevant updates.
• Evidence of training in infection prevention and control.
• Basic Life Support (BLS) certification or willingness to undertake training.
• Evidence of safeguarding training appropriate to the role.

Desirable

• General Practice Nursing qualification.
• Cervical Cytology Sample Taker qualification.
• Formal qualification in Diabetes, Respiratory or Cardiovascular Care.

Essential

• Experience working as a registered nurse.
• Experience of delivering evidence-based patient care.
• Experience in health promotion and patient education.
• Experience of maintaining accurate clinical records.
• Experience working within clinical governance frameworks.
• Experience with TPP Systmone (or in the absence of this, EMIS)

Desirable

• Previous experience within General Practice or Primary Care.
• Experience managing patients with long-term conditions.
• Experience in cervical screening.
• Experience delivering childhood and adult immunisation programmes.
• Experience undertaking NHS Health Checks.
• Experience participating in audit and quality improvement activities.
