Carry out annual diabetes reviews
Follow up appointments with patients
Liaising with secondary care team, GPs, administrators
Oversight of recalls, QOF and waiting lists
The practice nurse will work as part of the primary healthcare team providing the below services to the patients of the practice.
Chronic disease reviews - specifically Diabetes
Assist in and perform routine tasks related to patient care as directed by the GPs or other Clinicians
Maintain general tidiness and cleanliness of nurses and treatment rooms
Participation in administrative systems in the Practice
At all times there will be a need to maintain accurate records
- A duty to advise senior nurses of potential problems or errors within the range of assigned tasks
Any other delegated duties appropriate to the post

practice dedicated to serving a diverse population of around 16,600 patients. Our team
is comprised of experienced professionals, including GP Partners, Salaried
GPs, Nurse Practitioners, Paramedic Practitioners, Practice Nurses, and our
invaluable Reception and Admin team.
Qualifications
Essential
- Registered Nurse - adult
- Qualified in Phlebotomy
- Diabetes qualification
Desirable
- Chronic disease management - Asthma, COPD
