Physician Associate - Home Visit Service

• Conduct comprehensive clinical assessments in patients' homes.
• Obtain and document detailed medical histories.
• Develop and implement management plans within your scope of practice.
• Request and interpret appropriate investigations.
• Identify deteriorating patients and escalate concerns appropriately.
• Prescribe through delegated pathways where locally agreed and in accordance with current regulations.
• Work collaboratively with GPs, nurses, paramedics, pharmacists, and community teams.
• Maintain accurate and timely clinical records.
• Participate in clinical governance, audits, and quality improvement initiatives.
• Promote patient education, self-management, and preventative healthcare.

Essential

• MSc or Postgraduate Diploma in Physician Associate Studies or equivalent recognised qualification.
• Successful completion of the Physician Associate National Examination.
• Registration with the General Medical Council as a Physician Associate.
• Experience working in primary care, urgent care, community care, or a similar clinical setting.
• Experience undertaking patient assessments and formulating management plans.
• Experience working within a multidisciplinary healthcare team.
• Knowledge & Skills
• Excellent clinical assessment and diagnostic skills.
• Ability to recognise and manage acute and chronic presentations within scope of practice.
• Strong communication and interpersonal skills.
• Ability to work independently whilst recognising limitations and seeking support appropriately.
• Competent IT skills and experience using electronic patient record systems.
• Understanding of safeguarding responsibilities for adults and children.

• Previous experience within a Home Visiting Service.
• Experience in General Practice.
• Experience managing elderly, frail, or housebound patients.
• Experience conducting care home visits.
• Participation in clinical audit, service development, or quality improvement projects.

• Knowledge of community services and local healthcare pathways.
• Experience using EMIS, SystmOne, or similar clinical systems.
