An exciting opportunity has arisen for a Band 5 Physical Health Nurse to join our established multidisciplinary Healthcare Team as we expand into the ...

You will deliver direct physical healthcare to children and young people within Millennium Lodge as part of the multidisciplinary Healthcare Team. You will undertake physical health assessments, screening and clinical observations; recognise and appropriately escalate abnormal findings or deterioration; contribute to individual healthcare plans; and undertake clinical procedures within their competence.

The role will support immunisation and vaccination programmes, sexual health interventions, health promotion, long-term condition monitoring and visiting GP/primary care clinics. You will work collaboratively with the Band 6 Physical Health Practitioner, GP, wider MDT, residential colleagues and external healthcare providers to ensure continuity of care. You will maintain accurate clinical records, contribute to safeguarding, audit, quality improvement and supervision, and deliver care using a child-first, trauma-informed and non-stigmatising approach.

You will work flexibly across commissioned hours, including early and late shifts, and contribute to the weekend and Bank Holiday on-call rota for new admissions.

Qualifications

Essential

  • Registered Nurse with current NMC registration
  • Evidence of continuing professional development
  • Willingness to undertake additional training and competency development relevant to the physical healthcare needs of children and young people within a secure setting

Desirable

  • Registered Adult Nurse or Registered Children's Nurse
  • Additional training relevant to physical healthcare, young people's health, sexual health, immunisation or long-term conditions
  • Practice supervisor/ assessor preparation or willingness to undertake this following appropriate experience

Experience

Essential

  • Experience of delivering direct nursing care
  • Experience of undertaking physical health assessment and/or clinical observations
  • Ability to recognise abnormal findings and appropriately escalate concerns
  • Experience of communicating effectively with patients/service users, families/carers and other professionals
  • Experience of working as part of a multidisciplinary team
  • Evidence of maintaining accurate clinical documentation

Desirable

  • Experience of working with children and young people
  • Experience within primary care, community nursing, paediatrics, urgent care or another physical healthcare environment
  • Experience within mental health, learning disability, youth justice or secure services
  • Experience supporting people with complex health and psychosocial needs
