Undertaking a patient facing role, you will work alongside GPs, clinical pharmacists, nurses
and other healthcare professionals,helping to optimise medicines use, improve
patient outcomes, and support the delivery of high-quality care across our PCN
population.
You will be supporting the PCN in the achievement of its targets relating to IIF, CQC and QOF.
You will leadimprovements to maximise safe, cost-effective best practice in prescribing
to improve the quality of patient care.
You willsupport the Primary Care Network (PCN) practices by working within
professional boundaries as part of a multi-disciplinary team, to ensure the effective and efficient use of medicines,
and deliver key objectives of the PCN DES across the Network.

Experience
Essential
- Working in a patient facing role
- Working in a pharmaceutical department
- Experience with medication reviews
- Experience of medicine optimisation and reconciliation
- Working with community pharmacies
- Working in a role involving administration e.g. data checking and charging
- Working in a team as a team member
- Working with multidisciplinary teams
Desirable: - Working within Primary Care/General Practice
Skills and Knowledge
Essential
- Good IT skills including the Microsoft Office suite.
- Knowledge of shared care protocols
- Leading improvement skills
- Consultation skills
- Clinical Audit skills
Desirable: - Knowledge of PCNs; - Knowledge of QOF
Qualifications
Essential: - Registered as a Pharmacy Technician with the General Pharmaceutical Council (GPhC); - GCSE grade A to C or equivalent in English & Maths
Desirable: - Undertaking or qualified through an approved training pathway eg Pharmacy Educational Pathway (PCPEP) or Medicines Optimisation in Care Homes (MOCH)
