PCN Care Coordinator

The role of the CareCoordinator/Admin Assistantin the Primary Care Network is to support our multi-disciplinary clinical team to ensure that our patients, particularly those who are vulnerable or have complex needs are able to access services which meet their needs.

Key responsibilities and tasks

pilot work across the PCN

2. To support the work of the PCN multi-disciplinary team (MDT) to enable

them to provide high quality patient care.

Connexus Primary Care Network brings together five GP Practices

serving 50,000 patients in the Brislington, St Annes, Knowle, Totterdown and

Stockwood areas of South Bristol. The

PCN is located in

South Bristol locality of BNSSG and employs a small staff team that is

responsible for delivering a range of services under contract with NHS

England.
