Are you looking for a varied role where no two days are the same? Do you enjoy meeting interesting people from different walks of life on a daily basi...
You will work in a multidisciplinary team of nurses, therapists, and self-management facilitators, providing care to patients in their own home.
You will need to assess and respond to the changing needs of the patient, working within your own limits and seeking support from the wider clinical team where appropriate.
You will provide patients with clinical support for example administering medications, undertaking wound care and carrying out observations.
Please note this role does require a full driving licence and use of your own vehicle which is essential to this job role.
