Mental Health Practitioner

The main duties of a Mental Health Community Practitioner within

Our team includes (but not limited to)

Completing assessments on patients

referred within the Worcester City Locality. Referrals would typically come

from GPs, however, may also come from internally within the trust or other

agencies.

Holding a caseload of patients

with a range of mental health illnesses and providing treatment to these

individuals using evidence-based interventions as appropriate.

To develop care plans with

patients and the MDT, to the utilise these care plans to support them in

recovery.

To be able to risk assess and risk

manage patients in response to their current needs.

To take part in the duty system

and act appropriately when dealing with patients who are experiencing

difficulties in the absence of their allocated worker.

Receive clinical and managerial

supervision regularly, and to also supervise and support junior members within

the team.
