To provide
clinical nursing service and support to other professionals and to patients of the practice including treatment, screening, chronic disease management and advice within the professional competence of the post holder. Support and work effectively within the clinical team.
The successful applicant will
Provide clinical leadership to the nursing team and support
delivery of high-quality patient care.
Lead the management of patients with long-term conditions
using evidence-based practice.
Support and develop nurses, nursing associates, healthcare
assistants and students through supervision and mentorship.
Act as Infection Prevention and Control Lead, ensuring
compliance with national and local standards.
Deliver routine nursing services including chronic disease
reviews, immunisations, vaccinations, cervical screening and health promotion
activities.
Contribute to quality improvement initiatives, clinical
audits and service development projects.
Support achievement of Quality and Outcomes Framework (QOF)
and enhanced service targets.
Work collaboratively with GPs, PCN colleagues, community
services and wider healthcare partners to improve patient outcomes

forward-thinking GP practice in the west end of Newcastle Upon Tyne based in a
single site premises. At Prospect Medical Group we aim to provide quality

We currently have a patient list size of around 14,000. We
are highly involved in our PCN (Inner West Newcastle) and through this we
benefit from pharmacists, physiotherapists, psychology, paramedics, mental
health workers and social prescribers who work also work for the practice.
Our collective purpose is to provide the best care to our
patients.
We use SystmOne clinical system.
Experience
Essential
- Registered Nurse with current NMC registration.
- Minimum 2 years post registration.
- Experience working within general practice or primary care.
- Experience of chronic disease management
- Experience working autonomously and within multidisciplinary teams.
- Evidence of leadership, supervision or mentorship of clinical staff.
- Knowledge of infection prevention and control standards. Knowledge of Quality Outcome Framework (QOF)
- Excellent communication skills.
- Commitment to continuous professional development
- Evidence of appropriate knowledge base.
- Competent basic IT skills.
- Evidence of problem-solving and decision making.
- Team Work
- Ability to be reflective, accept criticism and act constructively.
- Chronic disease management qualifications
Desirable
- Basic/advanced certificate of life support.
- Uses initiative.
- Willingness to undertake further study.
- Understanding of clinical governance, safeguarding and quality improvement.
- Insight and understanding of current issues in nursing and the NHS
- Knowledge of SystmOne
- IPC Lead qualification
- Cervical screening qualification
- Demonstrate an understanding of audit.
- Adult and childhood Immunisation training
Qualifications
Essential
- Registered Nurse with current NMC registration.
- Qualified to offer the management of chronic diseases.
- Experienced Practice Nurse seeking progression into a clinical leadership role, or an existing Clinical Lead Practice Nurse.
- Applicants must have the right to work in the UK. Please note that this role is not eligible for Skilled Worker visa sponsorship
Desirable
- Qualified to offer Cervical screening certificate
- Qualified to offer childhood immunisation
