LD Crisis Response+Intensive Support Practitioner

It is an ideal role for someone looking to further develop their clinical skills, knowledge and competencies while being part of a passionate and supportive team dedicated to improving outcomes for people with learning disabilities and complex needs.

Triaging new referrals from a number of sources including referral directly from patients, carers or professionals.

Provide clinical assessment, support, planning and oversight for patients who are in the community at risk of admission and/or placement breakdown.

Offer training and advice to carers, providers and health and social care staff specifically in relation to meeting the individuals needs and through their Positive Behavioural Support Plan.

Provide specialist advice and support to people to enable them to remain in the community and prevent, if possible, in-patient admissions.

Monitor/ support utilising our pathways including the Mental Health Pathway and Positive Behavioural Support Pathway until there is sustainable evidence that the risks are manageable/ reduced and if required handing over to LD Community teams for longer term monitoring.
