Junior Specialist Doctor - Higher - Elderly Care

We would be keen to work with successful applicants to design rotations that would provide exposure to the full range of Geriatric and General (internal) medicine to suit their training needs and career aspirations. We envisage that the JSD (Higher) would act as a stepping stone into specialty training in Geriatric and General Medicine. The candidate will also contribute to the General Medicine rota where they will carry out roles in line with a middle grade in medicine. This will include leading the acute medical take, reviewing and managing acutely unwell patients in ED, AMU and wards and leading the EMRT team.

Appointees will gain experience in all medical procedures and will also work within the medical ambulatory care. This post in Elderly Care may involve in SpR rotations within Acute Frailty, General Geriatrics, Orthogeriatrics and Surgical Liaison. The successful candidate will have the opportunity to work alongside Geriatricians with a special interest in the above fields.

The Care of the Older Adult medicine Department is part of the Admitted Care Directorate of the Group of Medicine and Emergency Care. The Older Adult Medicine wards provide acute care with provision for further rehabilitation if required at various intermediate care sites in the locality.

The department provides a range of specialist geriatric services across the Trust. Our established Orthogeriatric service works closely with Trauma & Orthopaedics, providing geriatrician input for all patients with neck of femur fractures and a liaison service for other older orthopaedic patients. We also have a developing Surgical Liaison Service, providing geriatrician input to older patients under General Surgery, supporting recovery and safe, timely discharge. These services offer opportunities for multidisciplinary working across acute and surgical specialties alongside our wider frailty and geriatric medicine services.

The team currently operates a front-door frailty intervention service within a Frailty SDEC, providing specialist assessment and management for frail older patients presenting to the Emergency Department. This provides an exciting opportunity to work within a specialist multidisciplinary team and contribute to improving the care and outcomes of older people at the front door.
