The post holder is responsible for the delivery of primary care nursing services to the Practice population. Supported by the Lead GP within the Pract...

Patient assessment and clinical care assessing patients, taking medical histories, checking vital signs, and identifying health concerns. Chronic disease management monitoring and supporting patients with conditions such as diabetes, asthma, COPD, hypertension and cardiovascular disease, CKD

Immunisations and vaccinations administering routine childhood, adult and travel vaccinations and maintaining accurate records. Ordering and responsible for immunisation and undertaking audit and checks.

Health checks and screening carrying out NHS health checks, cervical screening, cardiovascular risk assessments and other screening programmes.

Wound care treating wounds, changing dressings, removing sutures/staples and providing advice on wound management.

Blood tests and investigations taking blood samples and other specimens, performing relevant tests and following up results according to practice procedures.

Medication-related care supporting patients with medication management, monitoring treatment, and working within prescribing or patient-group-direction arrangements where appropriate.

Infection prevention and control following appropriate infection-control procedures and maintaining a safe clinical environment. To be part of annual Infection prevention and control audit.

Team working working closely with GPs, healthcare assistants, paramedics pharmacists, district nurses and other members of the multidisciplinary team.

Framework

The

post holder should be a Registered Nurse who wishes to consolidate learning

from registration and wishes to put a range of skills into practice to develop

a portfolio of competencies relating to Primary Care. The role allows the

post-holder to experience working in primary care and achieve competencies

related to that role with the opportunity to complete a variety of CPD courses

relevant to primary care. The post-holder should have a desire to take on more

responsibility and structured learning. More in Job Description attached

Qualifications

Essential

  • Registered Nurse

Desirable

  • Practice Nursing Specialist qualification or willingness to work towards

Essential

  • Excellent communication (verbal and written) and interpersonal skills.
  • Able to work effectively unsupervised, independently and as part of a team.
  • Ability to deal sensitively with patients and colleagues at all levels and disciplines.
  • Able to work accurately and reliably under pressure in a busy environment.

Desirable

  • Able to adapt to change.
  • Working knowledge and proficient use of Clinical and Non-Clinical IT Systems.
  • Good organisational skills with a proven ability to organise own workload and to use own initiative.

Experience

Essential

  • Post registration experience
  • Experience of taking medical histories and completing episodes of patient care
  • Experience in chronic disease management or willing to work towards
  • Experience of health promotion and patient education

Desirable

  • Experience of working in a GP Practice
