*Internal MPFT Only* - Assistant Practitioner

The Assistant Practitioner provides rehabilitation assessment and treatment for patients following stroke, supporting recovery, discharge planning and independence. The role involves managing a delegated caseload, implementing therapy programmes, working closely with multidisciplinary teams, maintaining clinical records, and contributing to service improvement and seven-day service delivery.

We are looking for an enthusiastic and motivated Assistant Practitioner to join our Stroke Rehabilitation Team. The successful candidate will work closely with therapists and the wider multidisciplinary team to support patients to maximise their independence and achieve their rehabilitation goals.

This is an excellent opportunity for an experienced rehabilitation support worker or assistant practitioner who is passionate about delivering high-quality, person-centred care and helping patients achieve the best possible outcomes.

Qualifications

Essential: - Level 2 or above / equivalent; - GCSE maths & english or level 4 functional

Desirable: - Relevant training

Experience

Desirable: - Previous experience of working in a community/Hospital setting; - Evidence of multi-disciplinary team working

Skills

Essential: - Have evidence to show a good range of prior clinical experience; - Understanding of evidence based practice
