As a Healthcare Assistant within the Community Nursing Service at HCRG Care Group, you will be a key team member providing high-quality care to patients in Dartford and North Kent. This role involves delivering compassionate, person-centred care, supporting various healthcare needs including wound care and diabetes management, and promoting patient independence and wellbeing. The position is based at Livingstone Community Hospital, which offers excellent transport links and proximity to Bluewater Shopping Centre. You will have access to modern technology for remote documentation, reducing travel time and enabling more hands-on patient care. About the RoleBased at Livingstone Community Hospital, Dartford, you'll work as part of our Community Nursing Team, supporting patients to receive high-quality care in their own homes and community clinics.

This is a varied and rewarding role where you'll work alongside Community Nurses to provide compassionate, person-centred care that promotes independence, wellbeing and dignity.

Our teams are equipped with modern technology, enabling you to access patient records and complete documentation remotely, helping you spend more time delivering care and less time travelling back to base.

Located just off the M25, Livingstone Community Hospital benefits from excellent transport links and is within easy reach of Bluewater Shopping Centre and local amenities.

Main ResponsibilitiesWorking closely with Community Nurses and the wider multidisciplinary team, you will:

Deliver high-quality care to patients within their own homes and community clinic settings
Support patients with a range of healthcare needs, including wound care, diabetes management and end-of-life care
Undertake delegated clinical activities within your scope of competence
Monitor and report changes in patients' conditions to registered clinicians
Promote patient dignity, independence and wellbeing at all times
Accurately document care provided using electronic patient record systems
Develop positive working relationships with patients, families and healthcare professionals
Contribute to preventing unnecessary hospital admissions through effective community-based care
Support the continual improvement of services through shared learning and best practice

About YouWe're looking for a compassionate, motivated and organized individual who is passionate about delivering outstanding patient care.

To be successful in this role, you'll have:

An NVQ Level 3 in Health & Social Care, Care Certificate plus relevant experience, or equivalent healthcare qualification and experience
Previous experience working in a healthcare, community or care setting
Strong communication and interpersonal skills
Excellent organizational skills and the ability to manage a varied workload
Confidence using IT systems, including Microsoft Outlook, Word and Excel
The ability to work independently and as part of a multidisciplinary team

Access to a vehicle for work purposes
Ability to travel across the service area in a timely manner

Qualifications

Essential

An NVQ Level 3 in Health & Social Care, Care Certificate plus relevant experience, or equivalent healthcare qualification and experience; previous experience working in a healthcare, community or care setting; strong communication and interpersonal skills; excellent organizational skills; confidence using IT systems, including Microsoft Outlook, Word and Excel; ability to work independently and as part of a multidisciplinary team; full UK driving license and access to a vehicle for work purposes.
