Personalised behaviourchange coaching Provide structured onetoone coaching to help patients identify goals, build confidence, and make sustainable lifestyle changes.
Practicebased patient support Deliver facetoface sessions at Elm Tree Medical Centre and other sites, including wellbeing assessments, care planning, and followup reviews.
Communitybased engagement Work offsite across local venues, voluntary organisations, and social groups to connect patients with appropriate community resources.
Group education programmes Facilitate group sessions on topics such as healthy living, physical activity, stress management, and social connection.
Integrated multidisciplinary working Collaborate with GPs, nurses, social prescribers, mental health practitioners, and other allied health professionals to ensure coordinated patient care.
Demonstrable commitment to improving health outcomes and reducing health inequalities.
Strong communication skills with the ability to motivate, encourage, and build rapport with patients.
Experience working with diverse communities and individuals with varying levels of need.
Ability to work independently across clinical settings and community environments.
Understanding of behaviourchange approaches such as motivational interviewing or coaching models.
Confidence in maintaining accurate records and adhering to NHS policies, safeguarding procedures, and dataprotection requirements.

Elm Tree Medical Centre is a friendly and welcoming GP practice serving a growing population of approximately 19,000 patients. We are a busy, forward-thinking practice with a diverse multidisciplinary team, including 5 partners, working collaboratively to provide high-quality, patient-centred care.
