Health and Social Care Officer, Nottingham City £19.20 per hour The role is to process and triage adult social care referrals at the main front door to social care. This role is fully phone based, taking referrals over the phone and via online referrals. The role includes processing requests and appropriately screening referrals for care and support, occupational therapy equipment and safeguarding referrals.
