Health and Social Care Coordinator Salary: £28,000 - £30,000 | Location: South Tyneside | Type: Full-time, Permanent Reed is recruiting on behalf of a values-led care organisation for a Health and Social Care Coordinator to support the day-to-day operation of services for adults and children with learning disabilities. This varied role combines care coordination, administration, compliance, and quality assurance, helping to ensure safe, effective, and person-centred support. Key Responsibilities

Coordinate daily service delivery, staffing schedules, and rotas.Maintain accurate care plans, risk assessments, and care records.Ensure compliance with CQC regulations, safeguarding requirements, GDPR, and health & safety standards.Support audits, quality assurance activities, and continuous improvement initiatives.Build positive relationships with service users, families, healthcare professionals, and external agencies.Investigate, record, and monitor incidents, complaints, and safeguarding concerns.Provide direct care and practical support when required. What We're Looking For

Previous experience in health and social care coordination, administration, or support services.Knowledge of care planning, safeguarding procedures, and CQC requirements.Strong IT skills, including Microsoft Office and electronic care management systems.Excellent organisational skills with the ability to manage competing priorities.Full UK Driving Licence and willingness to undergo an Enhanced DBS check.
