  • Provide personalised, person-centred support to patients, helping them identify their needs, goals and priorities.

The Social Prescribing Link Worker will support patients to address the wider social, emotional and practical factors that affect their health and wellbeing. The post-holder will work with patients on a one-to-one basis to understand what matters to them, identify their priorities and goals, and connect them with appropriate local services, community groups, activities and voluntary organisations.

The role will promote independence, self-management and wellbeing while helping to reduce social isolation, address health inequalities and improve access to community support. The post-holder will work closely with patients, families, carers, healthcare professionals and community organisations to provide a coordinated and person-centred approach.

  • Undertake holistic assessments with patients to identify their social, emotional, practical and wellbeing needs.
  • Work with patients to co-produce personalised support plans based on their individual priorities, interests, strengths, values and goals.
  • Provide information, advice and signposting to relevant local, regional and national services, community groups and activities.
  • Connect patients with appropriate support relating to mental wellbeing, social isolation, physical activity, employment, housing, financial difficulties, caring responsibilities and other wider determinants of health.
  • Support patients to overcome barriers to accessing services and encourage confidence, independence and self-management.
  • Provide continuity of support throughout the social prescribing journey, including appropriate follow-up to review progress and outcomes.
  • Where appropriate, accompany patients to community services or activities where this is necessary to support engagement.
  • Encourage patients to identify practical steps they can take to improve their own health and wellbeing.

Referrals and Partnership Working - Work closely with GPs, practice staff, healthcare professionals, social care, voluntary organisations and community services to support appropriate referrals.

  • Promote self-referral and actively engage with local communities to improve awareness and accessibility of the service.
  • Support people who may benefit from social prescribing, including those with mild to moderate anxiety, depression or low mood, long-term conditions, frequent healthcare use or wider non-clinical support needs.
  • Participate in multidisciplinary team meetings and contribute social prescribing solutions for patients with complex needs.
  • Provide appropriate feedback to referral agencies regarding patient progress, engagement and outcomes while maintaining confidentiality.
  • Attend relevant meetings, community events, stalls and exhibitions to promote social prescribing and strengthen local partnerships.

Community Development - Develop and maintain strong relationships with local voluntary, statutory, community and faith organisations.

  • Maintain up-to-date knowledge of local services, groups, activities and support available to patients.
  • Maintain accurate information on relevant services using internal systems and local information portals.
  • Identify gaps or barriers within local provision and share relevant information with colleagues and partner organisations.
  • Facilitate effective links between primary care, secondary care, community health services and voluntary and community organisations.
  • Promote volunteering and community participation as opportunities to improve wellbeing, confidence, skills and social connection.

Monitoring and Service Development - Maintain accurate records of patient contacts, assessments, referrals, support plans and outcomes in accordance with practice policies and information governance requirements.

  • Ensure social prescribing activity and appropriate coding are recorded accurately on clinical systems in line with current NHS guidance.
  • Provide monitoring information, outcomes and case studies as required by the Practice Manager.
  • Seek and respond to feedback from patients, referral agencies and partner organisations to improve the quality and effectiveness of the service.
  • Participate in audit, evaluation, service development and quality improvement activities.
  • Attend regular team meetings, supervision and clinical supervision where appropriate.
  • Complete statutory, mandatory and role-specific training and maintain ongoing professional development.

The post-holder will have access to confidential information relating to patients, carers, colleagues and the practice. All information must be treated as confidential and handled in accordance with practice policies, data protection legislation, information governance requirements and relevant NHS guidance.

Information must only be accessed or shared with authorised individuals where there is a legitimate professional reason to do so. The post-holder must maintain accurate, secure and appropriate records at all times.

SAFEGUARDING The post-holder must understand and comply with their responsibilities for safeguarding children, young people and adults at risk.

Any concerns relating to abuse, neglect, exploitation or risk of harm must be reported promptly through the appropriate safeguarding procedures. The post-holder must complete relevant safeguarding training and maintain an appropriate level of knowledge and awareness.

GENERAL The post-holder will work flexibly according to the needs of the service and undertake any other reasonable duties appropriate to the level and nature of the role.

We are a well-established training practice in a purpose built (newly refurbished) premises, located on the Haringey Green lanes.

The practice's aim is to provide our patients with an excellent standard of patient-centered care, with clinicians and patients jointly involved in all aspects of planning and delivering care to meet patients' health and well-being needs.

You will work with colleagues including a team of GPs, practice Nurse, Healthcare Assistant, Clinical Pharmacist, Physician Associate & MSK practitioner, administrative staff and a wide variety of other health professionals.

There is free parking available on site.

You must have your covid vaccines to join as a member of the practice.

Experience

Essential

  • Experience working in community development, adult health and social care, learning support or public health/health improvement, including voluntary experience.
  • Experience supporting individuals, families and carers in a relevant role.
  • Experience supporting people with mental health and wellbeing needs.
  • Experience working with the voluntary, community and social enterprise (VCSE) sector, including volunteers and small community groups.
  • Experience collecting data and using tools to monitor and demonstrate service impact.
  • Ability to work flexibly, both independently and as part of a team.
  • Understanding of confidentiality, safeguarding, lone working, information governance and health and safety procedures.
  • Ability to recognise when a persons needs are outside the scope of the role and require referral or escalation to an appropriate health professional or agency.
  • Good understanding of professional boundaries and the importance of working within the limits of the role.
