Frailty SDEC Nurse

As a key member of the Frailty SDEC team, you'll provide high-quality nursing care to older adults presenting with frailty syndromes. You'll work closely with Emergency Department colleagues to proactively identify and manage patients, contributing to comprehensive geriatric assessments and the creation of supportive management plans.

You will help promote admission avoidance, reduce hospital length of stay, and support early intervention for individuals living with frailty.

Experience

Desirable: - Experience on an SDEC or in an Emergency Department.; - Post-registration experience

Skills and Knowledge

Essential: - awareness of current professional nursing issues; - Awareness of the role of an SDEC and current issues.

Desirable: - Familiarity with the fast flow of an SDEC and discharging of patients.
