Job Title: Enhanced Clinical Practitioner
Hours: 24 hours per week, Mon - Fri
Based
at: Open Door and Quayside Medical Centre
£46,208 pro rata per annum, fixed salary point
An opportunity has arisen for an Enhanced Clinical Practitioner (Nurse/ Paramedic) to join our growing team, you will act as an autonomous practitioner working independently and in conjunction with other healthcare professionals within a range of primary care facilities to assess, diagnose and treat the conditions of those patients attending the service.
You will provide expert professional advice to patients,
carers and colleagues and ensure the maintenance
of clinical excellence and will undertake Clinical Practice at an advanced
level using expert knowledge and clinical
skills to deliver holistic care to
people accessing services.
You must be an independent prescriber and have experience working with a wider range of individuals in the community, in return you will be able to contribute to the development of clinical lead services, be supported to develop your knowledge and skills and receive ongoing support.

Qualifications
Essential: - Registered with the NMC/HCPC; - Degree level qualification; - Non-medical Prescriber; - Mentorship in clinical practice
Desirable: - Masters level qualification; - Teaching qualification

- Clinical assessment and decision-making skills, with the ability to recognise and respond to complex patients
- Proficient in interpreting diagnostic tools
- Ability to work autonomously
- Excellent time management and organisational skills
- Knowledge of Mental Capacity Act; safeguarding adults and children; clinical governance; risk management
- Excellent verbal and written communication skills with the ability to build rapport and trust with patients and multidisciplinary teams
- Knowledge of national guidelines and ability to research good practices solutions
- Knowledge of working within budget and understanding financial management
- Conduct investigations

- Experience of non-medical prescribing to diverse client groups
- Experience of enhanced clinical practice particularly in the community or outpatient setting
- Experience of working within Quality agenda and undertaking audit
- Experience of identifying and managing risk
- Experience of working with individuals presenting with complex and diverse health and social care issues
- Proven ability to effectively act autonomously amongst a diverse client group
- Experience of managing staff
- Able to effectively communicate highly complex information
- Evidence of case management
- Experience in partnership working across professional disciplines and service specialities
- Experience of working with the safeguarding Child and Adults agenda

- Experience of leadership role and change management
