This role is designed to support Digital Transformation within the Southport and Formby Primary Care Network (PCN). It will involve working centrally ...

Key areas of work include but are not limited to:

  • EMIS and Digital Systems Support
  • Collaboration with PCN management teams

and staff across internal and external teams to drive digital transformation

and service improvements

  • Patient Support and assisting practices with Health Inequality Reduction
  • Drive Service Efficiency by standardising processes and workflows
  • Project and Task Management

You will be working in a growing team consisting of both clinical and non-clinical colleagues, so teamwork and good communication skills are vital.

Duties to include the following;

Create and run searches using EMIS to extract and analyse

relevant data.

Develop and maintain templates, protocols, and concepts in

EMIS to streamline practice and PCN workflows.

Utilise digital clinical tools such as Ardens, iPlato, and

Accurx to enhance digital healthcare delivery across practices and the PCN.

Use Microsoft Office 365 tools for advanced analytics,

reporting and visualisation to improve and streamline service managements and

reduce dependence on manual reporting and ad hoc fixes.

Collaborate with PCN management teams and staff across

internal and external teams to drive digital transformation and service

improvements including working on identified projects to support with data

quality/reporting and evaluation.

Building and maintaining relationships with practices by keeping

them up to date on performance measures such as the Sefton Local Quality

Contract (LQC) and Investment Impact Fund (IIF). This will involve supporting

and monitoring performance and reporting this back regularly to key

stakeholders.

Collaborate with existing PCN services, to drive the digital

maturity across the network.

Attending relevant meetings for services across the

organisation.

Identify opportunities for service improvement through

digital opportunities.

Assist PCN practices in engaging with health inclusion groups

and reducing health inequalities, aligning with the PCN Health Inequalities Strategy

and the requirements of the local and national contracts. This may include

supporting practices to risk stratify their patients based on clinical need

and/or locally identified priorities.

Use population health intelligence and management tools to

proactively identify patients requiring targeted interventions and support.

Supporting practices with digital communications via Batch

Messages to Patients (SMS Platforms - e.g., Iplato, AccuRx)

Monitor and report about PCN and/or practice performance on

key measures such as cancer screening rates and other identified priorities

locally or contractual obligations.

Drive integrated working across the PCNs services to enhance

collaboration between practices and/or PCN teams

Promote efficiency and reduce duplication by standardising

processes and workflows.

Maintain and further develop existing systems and processes

in place.

Support the reduction of variation between practices,

ensuring consistent service delivery across the network.

Manage work and communication using Microsoft Office products

to ensure effective coordination and task tracking across projects. This

includes Word, Excel and SharePoint, amongst others.

Monitor and report on digital transformation progress: Track

the implementation and impact of digital initiatives across the PCN and provide

regular reports to management on improvements and areas for further

development.

Have the ability to work on both small and large-scale tasks

as and when required, effectively managing your own time and utilising support

available as required.

Southport and Formby PCN is a group of 14 GP Practices located in the Southport and Formby area who work together with other local organisations to improve the health and wellbeing for our local population of around 129,000 people.

Skills and Knowledge

Essential

  • Able to build network links with external organisations as an ambassador for the Primary Care Network
  • Ability to communicate effectively and professionally with colleagues from a wide variety of healthcare professions and diverse groups
  • Creating/Editing/Running Searches and Reports in EMIS
  • Analysing data in excel

Desirable: - Data visualisation in excel & power BI

Experience

Essential

  • Competent with EMIS clinical software
  • Experience in complex administrative procedures
  • Competent in the use of Microsoft Office applications
  • Experience of working in a healthcare setting
  • Experience in coordination or project roles
  • Knowledge of community health and social care systems

Qualifications

Essential: - Educated to GCSE or A Level standard in English and Maths; - Evidence of continued personal development

Desirable: - Enrolled in, or willing to undertake or previously completed the Personalised Care Institute training

Personal Qualities

Essential: - Desire to continually learn; - DBS Check; - Ability to travel to other sites around the Southport and Formby area
