Responsible for read
coding and linking to existing and new problems by marking it as a new
episode/on-going and decide the appropriate place to attach it in the patient
record. Responsible for coding
complex medical documents and summarising Medical Record Envelopes when
received into practice. Management of NHS Mail and daily
incoming mail, this includes scanning and attaching all patient-related
documents to patient records and distribution of other post appropriately. Ensure all patient
records are maintained appropriately working to Information Governance
standards and the provisions of Data Protection. Managing E-consults,
subject access requests, Insurance reports, death certificates and cremation
forms. Processing safeguarding
documents in line with the Affinity Care safeguarding administration protocol. Dealing with medical
record movement and the EDI links and queries from PCSE. Responsible for assigning
and dealing with unassigned tasks in a timely manner that have been received
from internal and external sources to the
appropriate members of the team or individuals in a timely manner. Support the team with Retinal Screening reports running
and saving. Support the team on management of PNL and E-Cards
on the Cervical Screening Management System CSMS). Provide support across
Affinity Care when required.

Qualifications
Essential
- An understanding, acceptance and adherence to the need for strict confidentiality
- An understanding of data protection legislation
- GCSE English Language, Grade C or above (or equivalent)
- Experience using medical systems
- General Practice or NHS experience
Desirable: - Coding and summarising experience; - Working knowledge of Microsoft Office Applications
