Your role will involve supporting Lead Practitioners with their caseload of patients. Your role will involve supporting patients with targeted interventions, which are time limited, as well as social and practical support to service users by building up relationships with them. This particular role has a specific remit to provide preventive work with service users under the NMHT to avoid crisis or admission and support transition back from the crisis team to the NMHT.

The Trust provides mental health and learning disability care for all ages across Sussex and for children and young people in Hampshire.

QUALIFICATIONS

Essential

  • NVQ 3 or equivalent, combined with a commitment to undertake continuing personal development

EXPERIENCE

Essential

  • Experience of working with people with mental health problems, ideally in the community
