Community Staff Nurse Rapid Response

Provide high-quality nursing care to our patients in our local community to keep them safe at home, prevent hospital admission and support hospital discharge home.

Assess, plan, implement and evaluate individualised patient care in the most efficient and clinically effective way, whilst maintaining the patient's privacy and dignity.

Provide advice and support to junior staff in the delivery of high quality care and actively support the Community Nursing sister/charge nurses in the management of the health care environment.

Implement / refer on to the relevant services as appropriate.

Work in close collaboration with the Integrated Community Teams/GP surgeries and local acute trusts.

Education / Qualifications

Essential: - Full NMC registered Nurse; - Evidence of recent relevant training

Desirable: - Experience of Community Nursing

Experience and Knowledge Required

Desirable: - Experience of long term conditions; - Experience of Leg Ulcer management and lower limb assessments; - Experience of end of life care

Skills and Attributes

Essential: - IT Skills

Desirable: - Previous experience of using SystmOne

Other

Essential: - Ability to travel across Derbyshire County visiting patients' homes and across the county to access meetings and training
