Qualifications

Essential: - Nurse & Midwifery Council (NMC) active registration appropriate to the role

Experience

Essential: - Experience of managing patients with long-term conditions; - Show an awareness of current health and social care policies

Desirable: - Previous experience of community health care.

Knowledge

Essential: - Ongoing professional development and knowledge suitable to this service area
