The Community Matron will play a central role in supporting our patients to remain independent, avoid unnecessary hospital admissions, and receive high-quality care in the place they call home.

This role is ideal for someone who is passionate about proactive, frailty-focused care and making a meaningful difference to the lives of older adults.

Clinical Care

  • Provide advanced clinical assessment, diagnosis and management of adults with complex, long-term conditions - including frail, elderly and housebound patients - in their homes, care homes and community healthcare settings.
  • Deliver proactive clinical care, including falls risk assessment, polypharmacy review, and anticipatory care planning.
  • Work closely with families, carers and social care teams to support older patients with complex needs.

Leadership & Coordination

  • Lead and coordinate clinical care pathways with GPs, community nursing, care homes, therapy teams, social care and voluntary organisations.
  • Support the development of proactive frailty identification and early intervention within the practice and PCN.

Essential

  • Registered Nurse with valid NMC registration
  • Independent Nurse Prescriber (V300)

Experience

Essential

  • Significant post-registration experience in community or primary care, managing complex caseloads
  • Experience of multi-agency, multidisciplinary working
