Community IV Nurse- Newcastle

As a Community IV Nurse, you will provide specialist nursing

care to patients in their own homes, supporting them to safely receive

treatment outside of the hospital environment.

You will manage your own patient visits, assess patients and

deliver prescribed IV therapies and Parenteral Nutrition, while identifying and

escalating any clinical concerns appropriately.

A key part of the role is helping patients become confident

and independent with their treatment. You will provide education, guidance and

reassurance to patients, families and carers throughout their homecare journey.

You will work closely with NHS teams, healthcare

professionals and our wider Clinical Homecare Nursing Team to provide a safe

and seamless transition from hospital to home and ensure continuity of care.

The role offers a varied working day, a high level of

autonomy and the opportunity to build meaningful one-to-one relationships with

your patients.

Team you will deliver safe high-quality and patient-centred nursing care to

patients in their own homes and community settings

Working in partnership with NHS teams and healthcare

professionals you will support patients receiving specialist Parenteral

Nutrition IV therapies and other prescribed clinical interventions at home

The role offers the opportunity to work autonomously while

remaining part of a supportive wider clinical team

Deliver safe high-quality patient-centred care in patients'

homes and community settings

Administer prescribed Parenteral Nutrition IV therapies and

clinical interventions in accordance with standard operating procedures

professional standards and individual care plans

Undertake patient assessments monitor progress and identify

changes in a patient's condition

Recognise and escalate clinical concerns appropriately

supporting safe and effective patient care

Support the initiation of treatment ongoing care and the

safe transition of patients from hospital to home

Educate train and empower patients families and carers to

safely manage treatment and develop confidence and independence

Provide ongoing clinical advice guidance and support to

patients throughout their homecare journey

Develop positive and professional relationships with

patients families carers and healthcare professionals

Work collaboratively with NHS colleagues Senior Homecare

Nurses Clinical Nurse Managers and wider healthcare teams to provide continuity

of care

Maintain accurate timely and confidential clinical records

in accordance with Clinical Governance requirements NMC standards and Inizio

policies and procedures

Work in accordance with relevant clinical policies

procedures professional standards and evidence-based practice

Contribute to patient safety quality improvement and the

ongoing development of the Clinical Homecare Nursing service

Support the achievement of relevant service and programme

performance measures

Maintain and develop clinical competence through mandatory

training continuing professional development reflective practice and

evidence-based learning

This is a community-based position requiring regular travel

to patients' homes

Visit locations and routes will vary according to patient

needs and service demand

Travel between patient visits is included within your

working hours

You will need to be comfortable travelling independently and

hold a current valid UK driving licence

You will be responsible for managing your working day and

patient visits effectively while maintaining close communication with the wider

clinical team

As a Registered Nurse you will be expected to maintain your

NMC registration and practise in accordance with the NMC Code

You will take responsibility for maintaining your

professional knowledge and clinical competence and participate in relevant

training development and revalidation activities
