As a Band 3 Healthcare Support Worker within the team, you will provide high-quality, person-centred care to patients in their own homes, supporting them to maintain their independence and wellbeing. Working under the direction of registered nurses, you will assist with personal care, monitoring and recording observations, wound care, catheter care, simple dressings, and other delegated clinical tasks. You will identify and report changes in patients' conditions, maintain accurate clinical records, and contribute to care planning. The role involves travelling between patients' homes, working collaboratively with the wider multidisciplinary team, and promoting dignity, respect and patient choice always. You will also support health promotion, infection prevention and ensure equipment is used safely in line with Trust policies.

Experience
Essential: - Experience of working with older adults in a health/social care setting. Be able to meet the travel requirements of the role.
Qualifications
Essential
- Level 3 qualification (e.g. NVQ 3, A-level) in Care or equivalent qualification or experience Or Level 2 qualification (e.g. NVQ2, GCSE Grade A-C) in Care and will be expected to undertake the relevant Level 3 qualification on appointment to post Will be expected to meet the Care Certificate standards within 12 weeks of commencing employment with the Trust
Knowledge
Essential: - Basic knowledge of physical/ mental health conditions; - A level of understanding of rehabilitation
Skills & Abilities
Essential
- Ability to communicate clearly and effectively in English in both verbal and written formats
- Ability to use IT confidently and competently in daily work
- Good numeracy skills
- Ability to cope with difficult/emotionally distressing/ challenging situations
- Good organisational and time management skills
Additional Criteria
Essential: - Able to meet the travel requirements of the role
