Clinical Lead – Neighbourhood Mental Health Team, Waverley Locality

1.To support the service manager in the effective operation of the Frailty, Later Life and Dementia service line.2. To ensure the service operates in a safe, effective & efficient way as part of the whole Runnymede community neighbourhood team.3. To manage complex cases, providing leadership and supervision of staff and ensuring high quality evidence-based patient care.4. Being part of the community neighbourhood clinical leadership team -working collaboratively to ensure people's needs are met and a reduced need for repeat assessments.5. Assess patients/clients; plans, implements care; provides highly specialist advice; maintains associated records.6. May liaise with other agencies in planning programmes of care and/or health and education programmes

Be part of the community neighbourhood (INT) leadership team and provide a supervisory/consultation role for staff both within the service line and INT when required.

To be responsible for the development and maintenance of effective clinical pathways within the service line and wider community INT team.

Support safe, efficient and effective high-quality transitions in and out of the service line. (For full details, please refer to the enclosed Job description).
