An exciting opportunity has arisen to join an established team of children's community nurses on a fixed term/secondment basis to cover maternity leav...
The post holder will be responsible for the management of their own caseload.
To provide the delivery of evidence based nursing care to children the community settings by providing clinical assessment, care planning, care delivery, care evaluation and clinical advice in accordance with the NMC Code of Professional Conduct.
To develop planned programmes of care to promote health gains and maximise independence in all types of children within the community (for example, home, schools or other establishments) as required
Using autonomous judgement and liaising with the Matron and/ or multi-disciplinary professional as appropriate, manage children's health needs in accordance with clinical guidelines, using own clinical judgement to recognise acute exacerbations of illness and to initiate appropriate nursing interventions.
Using autonomous judgement and liaising with the Matron and/ or multi-disciplinary professional as appropriate, manage a changing caseload of children and identifying when the child's condition warrants discharge from the service.
To implement and evaluate evidence based training programmes for families, carers, other professionals and the multi disciplinary team.

Experience
Essential
- Proven experience at Band 5 in acute Paediatric setting.
- Experience relating to area of practice
- Mentoring of students.
- Evidence of managing and improving patient experience
Qualifications
Essential: - Registered Children's Nurse; - Mentoring / equivalent qualification; - BSc Community Specialist Practice / Recognised course in relevant area of practice
