Associate Practitioner - Physical Health and Wellbeing Service

o To provide physical health monitoring and wellbeing interventions to people requiring physical health checks or when initiating new medication, within Community Mental Health and Clozapine services.

o To deliver quality care and support which enables clients to be supported throughout their care pathway whilst promoting health and wellbeing.

o To work alongside both registered and unregistered staff running clinics, supporting colleagues and facilitating groups.

o To communicate effectively at all times within the team, with other health care professionals and with the client and their family.

Qualifications

Essential: - Educated to NVQ 3/4-Health and Social Care; - Care certificate

Desirable: - Grade C or above in GCSE Maths and English (or equivalent).; - Foundation degree in Health and Social Care or related subject (or equivalent experience).
