Advanced Nurse Practitioner General Practice

• Undertake autonomous clinical assessment of patients presenting with a wide range of acute and ongoing health problems
• Take comprehensive patient histories and undertake appropriate clinical examinations
• Formulate differential diagnoses and develop appropriate treatment and management plans
• Request and interpret investigations where appropriate
• Prescribe medication within their competence and scope of practice, where appropriately qualified
• Provide advice, treatment, health promotion and preventative care
• Recognise and appropriately manage clinical risk, including identifying patients requiring escalation or referral
• Make appropriate referrals to other services and healthcare professionals
• Support the assessment and management of patients with long-term conditions
• Maintain accurate, timely and comprehensive clinical records
• Work collaboratively with GPs, nurses, pharmacists and the wider multidisciplinary team
• Contribute to the continued development and delivery of high-quality services within the Practice

• Act as the main clinical point of contact for the care home, providing continuity of care and developing an effective working relationship with the care home team
• Undertake a scheduled weekly ward round at the care home, reviewing residents and responding to clinical concerns raised by the care home team
• Provide appropriate clinical assessment, review and management of residents with acute and ongoing health needs

Essential

• Registered Nurse with appropriate Advanced Nurse Practitioner/Advanced Clinical Practice qualification
• Previous General Practice or Primary Care experience
• Experience of autonomous clinical assessment and decision-making
• Ability to assess and manage patients presenting with acute and ongoing health conditions
• Excellent communication and interpersonal skills
• Ability to work autonomously and as part of a multidisciplinary team
• Ability to work within professional competence and scope of practice
• Ability to establish an effective working relationship with the allocated care home and undertake the weekly ward round
