This will include complex discharge of patients directly from SDEC and identifying the patients for whom admission to an inpatient speciality is the most appropriate destination. It will require the post-holder to advocate for the needs of their patients and to be part of the multi-disciplinary team that adds real value to the patient experience and outcomes whilst delivering an efficient and effective service for the Trust and wider healthcare community.
